The Vatican's official rules provide for penalties for unauthorised overflight of the state's territory, including with a drone controlled from outside its borders, so without explicit, individual authorisation you should not plan a flight over the Vatican.

Unauthorised overflight of the Vatican is separately penalised. Decree No. DCCX provides for imprisonment from 6 months to 3 years or a fine from EUR 10,000 to 25,000.
The prohibition also covers an overflight carried out with a drone controlled from abroad, so taking off outside the Vatican's borders does not solve the legal problem.
In practice there is no public authorisation route for recreational flights or generally available civilian flight map for the Vatican. The starting point should be simple: without an individual permission, do not plan a flight over the Vatican State.
Any possible operation would require at least the individual consent of the Vatican authorities and practical coordination with the surrounding airspace of Rome and the rules in force on the Italian side.
